In order to access the computer generated forms, you MUST complete the Student Information section of the Excent Program.


TERMINATION FROM SERVICES

Needed to drop students from special education services.
Completed when:

Child no longer qualifies for services
Child graduates
Child moves from district


Requires team members and parent signatures.  Send original to Special Services
and include a Prior Written Notice.

PRIOR WRITTEN NOTICE

Necessary to make ANY changes in educational placement for a child.  This notice begins a ten-day timeline for the parents to respond to proposed actions.  See “Prior Written Notice” section of this handbook.  Send original to district office.

PROGRESS REPORTS

Special education students must have IEP progress recorded and sent home whenever general education student progress is recorded such as; report cards, semester/quarterly progress reports.  Copies of the progress reports must only be filed at Special Services at annual IEP due date and eighteen weeks after the annual IEP has been held.

EXTENDED SCHOOL YEAR FLOWCHART

Must be completed for initially placed students.  The date for completion is the date stated in the IEP for “to be determined by” in the ESY section.  See “Extended School Year” section in this handbook.  Send original to Special Services.

STATEMENT OF ELIGIBILITY

Completed to document eligibility categories for special education services.  Form is part of the Evaluation Conference Summary.  Requires team members and parent signatures.

NON-PLACEMENT STATEMENT

Completed when student DOES NOT QUALIFY for special education services after an initial or re-evaluation.  Requires parent signature.  Send original to Special Services.
 

STUDENT TRANSFER FORM

Completed when a student moves to another school within the district.  Send a copy to receiving school and original to Special Services.

CONSENT TO EVALUATE

Consent to Evaluate is used obtain parental written consent to perform any type of assessment to consider special education services. Consent also means that the parent understands the following parental rights to the evaluation that include:

The reasons for the need to evaluate
The general types of tests to be used
That the child will not be evaluated for special education services without the parents’ written consent
The right to request an independent educational evaluation
That more than one assessment will be used
That the child will be evaluated by a team, including a specialist in the area of suspected disability
The right to a due process hearing
The right to exercise limited control over other people’s access to the child’s educational records
The right to request an independent educational evaluation if the parent disagrees with the evaluation conducted by the district

Completed by a team member prior to:

Adding a related service

IEP ACCEPTANCE FORM

Completed when a student enters the district with an active IEP.  Services will continue without modifications.  Send copy to parent and original to Special Services.

IEP REVIEW

Completed to review an existing IEP for compliance.  All areas must be completed to meet compliance.  Send original to Special Services.

EVALUATION REPORT REVIEW

Completed to review an existing evaluation report for compliance.  All areas must be completed to meet compliance.  Send original to Special Services.

RELEASE AND EXCHANGE OF INFORMATION

Tool completed for communication between school district and outside agencies.  Requires parental consent.  Send original to Special Services.

PARENT QUESTIONNAIRE

Completed to gather relevant information from parent prior to an initial or re-evaluation.  Usually given when “Consent for Evaluation” is sent.

PARENT CONFERENCE REQUEST

Completed to invite parents to meetings for the purpose of discussing evaluation results, developing, reviewing, or revising a child’s IEP.  Notify parents of the meeting early enough to ensure that they will have an opportunity to attend; preferred practice is considered ten (10) days prior to the meeting date.

IEP ADDENDUM

Completed to add or delete IEP provisions to a current IEP.  Must include Prior Written Notice.  Requires parent signature.

Completed to record any parent contact including, but not limited to; phone calls, hand written exchanges, parent-teacher conferences and incidental visits.

BEHAVIOR INTERVENTION PLAN

Completed to outline specific behavior modifications for a special education student.

MANIFESTATION DETERMINATION REVIEW

Completed at a Manifestation Hearing.
